CGHS card beneficiaries who have not applied for plastic cards are requested to submit required documents at their respective Wellness Centres before June 30. A Vijay Kumar, Additional Director Central Govt. Health Scheme told this.
Application forms can be downloaded from cghs.nic.in. Applications forms along with required documents have to be submitted at their respective Wellness Centre.
